About filling your application form

You may fill in the application form only with either Adobe Acrobat or
the free Adobe Reader. If you do not have Adobe Reader, you
may download it from here. When filling the form, please be as
complete as possible. Note: your web browser or other viewer does
not have the form-filling feature that saves your data for sending to us.

Use the buttons at the end of the form to

• print a copy for your own records
• print a copy to post with your fee to the address
  at the top of the form
• submit the form by email (expect an autoresponse receipt by return)

Submitting the form by email helps a great deal - we will not have to
